How to Claim

In order to claim sick pay benefit simply let us know, by email, letter or telephone, within the required notification period and we will send you a claim form to complete and return to us along with original evidence of illness or accidental injury and earnings. A failure to fully complete the claim form in accordance with the guidance notes and supply evidence of earnings and incapacity could result in a loss of benefit.

When in claim evidence of continuing incapacity must be provided to avoid a loss of benefit. For further information on the claims procedure contact the Society on 0845 603 8085 or email customerservices@cirencester-friendly.co.uk.


For Members who have selected the 'houseperson' incapacity definition cover is restricted.

We reserve the right to obtain, where relevant, proof of income.
